To configure your attachments go to Marketing automation, then components in forms suite

Click add

PS: make sure you have created a category with the type: Asset

Build your form as explained here –> for quote request forms

A static section will also be available to obtain statistics on your attachments

In Form Builder, build your form,

In Information & settings of the form, add all the requested information

In embed code, format your form and

Save.
